What is the evaluation process?
STEP 1 – Contact Us
Contact us by phone (952-236-1267) or via the contact form.
STEP 2 – Complete Paperwork
Complete the intake paperwork via the patient portal.
STEP 3 – Schedule Appointments
Schedule the evaluation by phone. Most children will require one full day of testing. Caregivers will then meet with the evaluating clinician for a second appointment one week later to review results and discuss treatment planning.
STEP 4 – Attend Appointments
Attend the appointments. A legal guardian must be present with the child for the full day testing appointment. During the testing appointment we will conduct interviews and administer tests. The second appointment is a one-hour telehealth meeting with caregivers during which results and treatment recommendations will be reviewed.
STEP 5 – Written Report
Obtain the written report. We will send you a detailed neuropsychological evaluation report approximately 4-6 weeks after your final appointment.
STEP 6 – Implement Treatment
Implement treatment recommendations and watch your child gain the support and confidence they need to grow!
STEP 7 – Schedule Follow-Up Evaluations
We will contact you to schedule follow-up evaluations as recommended by your provider to ensure your child continues to thrive. You can also contact us for re-evaluation prior to major life transitions, when growth plateaus, or when new obstacles emerge. We will be happy to help!
